Party Affiliation Declaration Form

In New Jersey, any independent or unaffiliated person may vote in a primary election by simply filling out a simple form called a Party Affiliation Declaration. (A person may change their party affiliation back to unaffiliated or independent the day after the Primary.) The form can be filled out when you go to the polls to vote on June 3rd, or you can submit it earlier to your Commissioner of Registration or Superintendent of Elections.

VOTING BY MAIL

New Jersey also allows you to Vote-by-Mail and include the Party Declaration Form. Click here for more information on Voting by Mail.

To Vote by Mail, please download each of the forms below and return to the addresses listed for your county. The County will then send you a ballot in the mail for you to return. PLEASE NOTE: THIS FORM MUST BE RETURNED TO YOUR COUNTY CLERK'S OFFICE NO LATER THAN 3PM ON TUESDAY, MAY 27th IN ORDER FOR YOU TO RECEIVE YOUR BALLOT. If you email us at the bottom of the page that you intend to Vote-by-Mail, we can ensure that your request was received and the ballot mailed to you.

If you do not receive your ballot within one week of your request, please email us ASAP at vbm@robandrewsforchange.com so that we can help you get your ballot.
